HR Form of the Week: ADA Medical Certification

This week, the HR Form of the Week is the ADA Medical Certification.

For clients with access to the Forms Library, see the document ADA Medical Certification in the “Americans with Disabilities Act (ADA)” > “Certification, Request, Letters, Forms” subfolder.

This form must be completed by an employee’s physician and is used to verify that an individual has a disability covered under the Americans with Disabilities Act (ADA) and if any workplace accommodations are needed. We recommend attaching the employee’s job description to the ADA Medical Certification.
